The SHA waiting period in Kenya: when your contributions actually cover you (2026)

When SHA cover starts after you register or resume paying. The Lipa Pole Pole 4-month upfront rule, the reactivation lag on Paybill 200222, and the ECCIF emergency exception.

What a "waiting period" means under SHA

A waiting period is the interval between the point you enrol (or resume contributing) and the point SHA will pay a claim on your behalf. Under the old NHIF regime, a 60-day waiting period for new individual members was widely reported. Under SHA, no equivalent single figure is set out plainly on sha.go.ke at the 26 August 2026 check. In practice there are three separate scenarios, each with its own timing.

Scenario 1: salaried Tier 1 member, first contribution

Salaried members contribute 2.75% of gross salary via employer deduction, remitted to SHA by the 9th of the following month. Cover follows the successful posting of that first contribution to your SHA member record. The employer file drives most of this; the member has little direct control over timing. See SHA for employers: what you must deduct and remit and is SHA deducted before or after tax.

If a first-time salaried member needs care shortly after their first payslip, the safe move is to check status via *147#. If status shows inactive because the employer's first remittance has not yet posted, ECCIF still covers qualifying emergencies.

Scenario 2: informal-sector member using Lipa Pole Pole

Lipa Pole Pole is SHA's 24-month instalment plan for informal-sector members. SHA's own page [Source 1] sets out:

  • Duration: 2 years.
  • Upfront: the first 4 months' premium paid at enrolment via Paybill 200222 (account = National ID).
  • Repayment: the remaining 20 months via M-PESA Ratiba or *147#/*254#.

The same SHA page also states, under Key Benefits, that "Full annual cover begins immediately upon enrolment". Field reporting on Lipa Pole Pole has described members needing to complete the 4-month upfront before cover activates in practice, which is not the same statement. Where SHA's own page and reported field practice do not clearly agree, the reliable move is to verify your specific status via *147# or 0800 720 601 before assuming cover. Full walkthrough: SHA Lipa Pole Pole explained.

Scenario 3: existing member clearing arrears

A member in arrears is inactive. Once the outstanding contribution (plus any 2% monthly penalty under section 27(6) of the Social Health Insurance Act 2023; see SHA arrears and the 2% penalty) is paid to Paybill 200222, the reactivation typically posts to your record within 24 to 72 hours as M-PESA reconciles to SHA.

This is a reconciliation lag, not a policy waiting period. Once the payment posts, you are active with immediate effect. The lag is what matters at the admissions desk: pay on Monday for a Thursday admission and you should be fine; pay on Wednesday for a Thursday admission and you may not be. If cover has not activated by the time you need care, follow SHA payment not reflecting: what to do. Verification via *147# or eligibility.sha.go.ke.

The ECCIF exception, in every scenario

The Emergency, Chronic and Critical Illness Fund covers [Source 2]:

  • Ambulance evacuation from the incident location to the nearest medical facility.
  • Accident and emergency services: severe burns, head injuries, fractures, seizures, unconsciousness, snake bites, rabies exposure.
  • Critical illness care: ICU, HDU, NICU, PICU, burns unit admissions for life-threatening conditions.
  • Palliative care for chronic illnesses.
  • Chronic illness management including diabetes, cancer, and mental health.

ECCIF is publicly funded, not from member premiums, and emergency access does not depend on the member's SHA contribution status. This is the most important thing to know about waiting periods: an emergency is never a moment when SHA arrears can lawfully prevent treatment. See SHA for chronic and emergency conditions.

If you need care during a waiting period

  1. Is it an emergency? Present at the nearest contracted Level 4 to 6 facility and invoke ECCIF. Do not delay for arrears clearance.
  2. Is it non-urgent but planned? Delay if possible until your status activates. Check *147#. If you cannot delay, pay out of pocket at a public facility (typically far lower than private retail) and keep every receipt.
  3. Are you using private insurance in parallel? Present private cover for the planned care. SHA is not the only cover source; a private plan or corporate health cover often works while SHA activation catches up.
  4. Do you know a member of your household is active? Under household coverage, a principal member's active status covers spouse and dependants. See SHA household coverage: who is covered. If any household member is active, the household may be covered even if you personally show inactive.

Practical timing checklist

  • Register at least a week before any planned admission via afyayangu.go.ke or *147#. See how to register for SHA.
  • Pay arrears at least 5 to 7 days before the admission date to allow full reconciliation.
  • Confirm status the day before admission via *147# or eligibility.sha.go.ke. See how to check your SHA payment went through.
  • Take your National ID, SHA member number, and (if possible) a printed active-status screenshot to admissions.
  • For Lipa Pole Pole, make sure the 4-month upfront is complete before assuming cover for elective admissions.

If you are in doubt

The one call that resolves waiting-period questions in practice is 0800 720 601. Have your National ID and SHA member number ready. The agent can confirm your active status, the date it activated, and any restrictions. Where the SHA-published rules and field practice do not clearly align, this call is the authoritative answer.
