What SHA is, in one paragraph
The Social Health Authority replaced NHIF on 1 October 2024 under the Social Health Insurance Act 2023. SHA operates three funds under one authority: the Primary Healthcare Fund, the Social Health Insurance Fund (SHIF, where the 2.75% contributions land), and the Emergency, Chronic and Critical Illness Fund. Every Kenyan resident must register at sha.go.ke/register and contribute monthly.
Tier 1 vs Tier 2, the split that matters
SHA classifies contributors into two tiers. Tier 1 covers salaried employees: the employer computes 2.75% of gross pay each month, deducts it before PAYE, and remits to SHA via employers.sha.go.ke by the 9th of the following month (per consultant guidance sourced from SHA). Tier 2 covers everyone else, freelancers, business owners, informal sector workers, farmers, hustlers. Their premium is set through a Means Testing Instrument that runs inside afyayangu.go.ke, and the minimum remains KES 300 per month regardless of assessment outcome.
Where to pay
M-PESA paybill 200222, account is your National ID number. USSD *147# for members without smartphones. Full walkthrough on our SHA paybill page, including the common failure modes (wrong ID format, portal sync lag, employer remittance not linked).
If your payment does not reflect
M-PESA confirmation but no portal update is the single most common issue with SHA payments in 2026. Full recovery playbook, including the 24-72 hour reconciliation window and when to escalate to the toll-free helpline 0800 720 601 or a physical SHA branch, is in our payment not reflecting guide.
How the Means Testing Instrument actually works
The instrument is a 30 to 45 minute questionnaire that runs at afyayangu.go.ke after you register at sha.go.ke/register. It captures household composition, housing type (owned, rented, shared, temporary), access to water and electricity and sanitation, assets (livestock, land, vehicle, business), education level of the household head, and employment status. Deep dive with the honest limits on what can and cannot be predicted is in our SHA means testing guide.
